The Customer Information Form should be completed by:

• New Customers who have an MBE Quote # or Purchase Order
• Customers who have not purchased from MBE in more than 12 months who have an MBE Quote # or Purchase Order
• Customers who are applying for Credit Terms with MBE for an order over $1,000
• Customers who are requesting an increase in their credit limit with MBE
• Customers with major changes such as name changes, mergers, acquisitions, etc.
